The Assessment Process
To begin an assessment, it is critical that we know that we can provide you with the assessment that achieves your intended goals. This might include registering your child in a specific program, obtaining accommodations in school, or understanding the diagnosis for strategies, medication or treatment.
STEP 1: A free 15 minute consultation to understand your goals. Please contact 587-321-3213 to book a consultation with Lesley Xu. She will be able to provide information with regards to the best suitable assessment and the associated process to address your concerns directly.
STEP 2: If you decide to proceed with the assessment, the following will occur.
(a) Completion of an assessment consent form and a Release of Information (ROI) to contact a teacher (if required). You could also be provided a document to complete about your child's history (NEPSY-II).
(b) A parent/guardian intake meeting (in-person or virtual over video) to discuss your child in depth and review consents.
STEP 3: Assessment with Lesley and your child.
STEP 4: Lesley will contact your child's teacher to include their perspectives on your child.
STEP 5: Lesley prepares a comprehensive individualized report that will be debriefed with you once complete.
