Veteran Assessments and Treatment

VAC assessments provide a comprehensive evaluation of mental health challenges obtained during service. The medical questionnaire is structured to align directly with VAC’s Table of Disabilities. This ensures that the adjudicators have the exact data they need to calculate your entitlement and potential compensation.

How to Request a Medical Questionnaire
You should not simply download and fill out a questionnaire on your own before starting an application. Here is the standard procedure:

Start Your Application:

1. Begin your disability benefit application via My VAC Account or by mail.
2. Wait for VAC’s Instruction:
3. Once VAC receives your application, they will determine which specific questionnaire is needed (e.g., for Hearing Loss, PTSD, or Knee Conditions).
4. Receive the Form:
VAC will send the relevant questionnaire directly to you through your My VAC Account (secure messaging) or by mail.
5. Take it to Your Provider:
Book an appointment with your doctor or specialist. They will complete the form and return it to VAC, or give it back to you to upload

***VAC will reimburse your healthcare provider for a reasonable fee to complete these forms. You should not have to pay out of pocket for the completion of a requested questionnaire.

If treatment is recommended, we will connect you (if desired) with specialized care to help you heal. These supports are either funded through VAC directly or through PCVRS.
